Online Form: Water System Shutdown Request

DESCRIPTION:
Application to request water main shut down. 

To Request Water Meter Shutdowns Call: Public Utilities Customer Service 435-615-5335

 

REQUIREMENTS:

  1. Allow up to five (5) business days for processing the shutdown request.
  2. Obtain all applicable permits, including but not limited to: road/sidewalk closures, right-of-way (ROW), and building permits.
  3. The requestor is responsible for distributing written notice at least 48 hours in advance to all parties affected by the shutdown.
  4. Schedule shutdowns to begin between 8:00 AM and 9:00 AM, Monday through Friday.

PROCESS:

  • The request will be reviewed by the Water Department.
  • The Water Department will coordinate with the Requestor and/or General Contractor to confirm the schedule and procedures for the hot tap.
